How to renue PRC Nurse License in PRC Manila

Professional Regulation Commission Manila is located at P. Paredes St., corner N. Reyes St., Sampaloc, Manila.

THINGS NEEDED:
– 1 passport size photo with collar, white background and your name tag (just like as your photo that you submitted to the PRC on your first license)

– Your license number/ registration number

– 450 pesos for the fee

– black pen (so that you will not need to borrow from other people)

STEP 1 – Go to the CIVIL SERVICE INFORMATION WINDOW. It is located at the left as you enter the PRC entrance gate. Tell the clerk that you are for renewal of nurse license. He will give you a small rectangular form.

STEP 2 – Fill out the form that was given to you by the clerk. Name, Address, Date of birth, License number, mobile number and e-mail address. Then paste your passport size picture at the left side. (PASTE. DO NOT STAPLE)

STEP 3 – Go to window 15. It is located at the MAIN BUILDING. Give your filled out form to the clerk then he will be writing the time you submitted the form to him. Wait for your name to be called on window 17.

STEP 4 – When your name is called go to window 17. The clerk will give you a printed form of your information and scanned picture.

STEP 5 – Proceed to window 23 (CASHIER). Pay the 450 pesos. The cashier will give you your receipt.

STEP 6 – Wait for your name to be called again in window 17 for the issuance of your new license.

How to apply for NBI clearance in Las Piñas

The Las Pinas NBI Office transfered to it’s new location. It is at the parking lot of SM Center Las Piñas across Las Piñas Municipal Office.

Step 1. Visit the NBI website http://nbi.njis-ph.com fill out the form then print the NBI form.

Step 2. Go to the parking lot of SM Center Las Piñas. You will see the “Las Piñas Satellite Office” sign. That is the NBI office. Proceed to the man waiting at the long table in the ENTRACE. Give your filled out NBI form to him. He will write your number on that paper.

Step 3. Wait for your number to be called. Go to the CASHIER WINDOW to pay for the fee. 115 pesos for both LOCAL and ABROAD.

Step 4. Proceed to window 2 which just next to the CASHIER WINDOW. They will give you the receipt.

Step 5. Go to window 3. This is where they scan your finger prints and take your photo. They will inform you if you can get your NBI clearance right away. If not, they will inform you when to come back and claim your NBI clearance.
